This full-time position has a starting salary of £61,954 per-annum based on a 36-hour working week. 

 

We have a great opportunity to join our Frimley Park Hospital team as a Team Manager on a secondment or fixed term contract until 31st March 2027. For a secondment, please discuss with your current Line Manager before submitting your application. 

 

This is a hybrid role, working from the office base at least three days per week, in line with our Team Charter. Our service operates from 8am to 6pm, Monday to Friday, and 8am to 4pm at the weekend. This Assistant Team Manager role covers Monday to Friday.

About The Role 

You will lead a multidisciplinary team to deliver safe, timely and effective discharges for residents with adult social care and support needs, ensuring practice is consistently legal, ethical and aligned with national and local legislation, guidance and best practice. Your role will involve managing discharge activity from Farnham Community Hospital and Frimley Park Hospital, including timely transitions from the Discharge to Assess pathway within the community, while maintaining strong, collaborative relationships with health partners, professionals and wider stakeholders. You will communicate organisational change clearly, supporting the team to continue meeting service demands during periods of transition, and will provide direct line management to approximately five staff, with overall responsibility for team performance.

The team plays a vital role in delivering Surrey County Council’s Adult Social Care vision, particularly the home-first approach, strengths-based practice and the ambition to leave no resident behind. As the Team Manager you will have a significant impact on hospital flow and positive discharge outcomes, supporting NHS partners to provide the right care at the right time, while also ensuring services are delivered in a cost-effective way through the best use of internal resources and voluntary, faith and community sector provision. Based at Frimley Park Hospital and Farnham Community Hospital, this is an excellent opportunity to step into a leadership role within a supportive, high-performing acute hospital social care team, known for its collaborative culture, commitment to professional development and strong track record in delivering person-centred discharges. Working as one of five acute hospital teams within Adult Social Care, the service supports residents and families to access the right short-term support, care packages or placements on discharge, enabling assessment and recovery in the community and helping people regain independence as quickly as possible.

 

Your Application  

In order to be considered for shortlisting, your application will clearly evidence the following skills and align with our behaviours:

  • A professional Social Work or Occupational Therapy qualification with current registration to the relevant professional body
  • Significant post-qualification experience in adult social care, including management experience and a strong understanding of social care and health agendas
  • Sound working knowledge of Adult Social Care legislation, including the Mental Capacity Act, NHS Continuing Healthcare National Framework and safeguarding adults procedures
  • Excellent communication skills, with the ability to manage and lead teams, provide effective professional supervision and build strong working relationships
  • A proven track record of effective inter-agency and partnership working
  • A strong commitment to strengths-based practice, promoting positive outcomes, community inclusion, and personal choice and control

 

Surrey has both urban and rural areas and locality based social care workers will be expected to have a valid driving licence to drive in the UK, access to a vehicle and be willing to travel across a wide geographical area.
